Looking for a predictable payment for this years primary care health services?  This package meets your typical yearly health care needs at a locked-in price.  This is an ideal product for the high-deductible insurance client as well as the uninsured.  This could be a gift option for someone you care for.

Internal Medicine of Southwest Florida recognizes that not all persons have health insurance. Also, many insurance plans have high deductible payment requirements for their customers. We have agreed to provide a comprehensive package of services for those individuals who would like predictable annual health costs. This plan requires individuals to pre-pay on an annual basis. The group agrees to provide an annual physical examination, EKG, comprehensive lab package, and two additional office visits in a twelve month period. The fee for this package effective January, 2010 is $800.00*.  For those who prefer, a monthly payment of $66.85 is another payment alternative for the Comprehensive Care Package.

The package includes:
 
- An annual physical including one hour of clinical/physician time.
- Two additional 20 minute office visits - can be used for new acute problems, or additional counseling/follow-up.
- One EKG with interpretation.
- A comprehensive lab package including: complete metabolic lipid profile, LDL cholesterol, TSH (thyroid testing) and Urinalysis. Males over the age of 49 will receive a PSA (prostate cancer screening test).
 
Office visits and or tests not listed in the package will be provided at our normal rates.

Internal Medicine of Southwest Florida is provides a full Corporate Health Assessment with physical examination, interpretation and review of labs and screening tests. On site services that are presently offered for this service include:  Comprehensive lab testing including Advanced Lipid and Coronary Risk assessment blood-work, PSA (if male 50 and over), Thyroid function testing, Complete Blood Count, Comprehensive Blood Chemistry testing and Urinalysis.  A full hour of physician time for initial assessment. Other non-blood testing include EKG, treadmill stress testing, pulmonary function testing, hearing testing, metabolic caloric testing, carotid, aortic, and peripheral artery doppler screening, echocardiography (cardiac ultrasound).

Obviously testing would be done ahead of the physician interview and review time and thus would likely involve a 2 day procedure (day one for testing and a later date for review and interpretation appointment).  We do not have hotel or other living arrangements for out of town clients.
 
Dr. Kordonowy is also available for employee health education lecture services or to assist in designing other screening services for employees.
