*Physician charges for evaluation and management services are determined by a complex set of criteria established by Medicare. Based upon numerous factors such as: time, complexity, data ordering and review, amount of physical examination, etc, there are 5 allowable "Levels of Service". For the above reason's, we cannot quote exact costs for evaluation prior to your visit as the physician will determine the level of service provided after the actual services are provided. This is why our staff will quote a price range when asked rather than a specific dollar amount. New patient fees are higher due to the inherent increased time spent registering patient information, chart preparation as well as reviewing their entire past medical history, medications, etc. Tests and procedures provided are not included in the evaluation and management "Levels of Service" and thus add to charges/costs to the patient.

We provide a complete wellness evaluation and physical examination for the fees listed below. This is included in the Comprehensive Care Package.

** Basic lab studies include a Complete Metabolic Panel, Lipid Profile, LDL Cholesterol, TSH, and Urinalysis. A PSA is included in a male exam.
